Important New Patient Information
Our practice always welcomes new patients!
If you would like to register with our practice we ask you to contact our reception team on 028 90 792 729 or in person at reception. Then you will be able to arrange a time to suit and we will ask you to attend your first appointment for a detailed examination in order for our dentist to record your present dentition. Your allocated dentist will discuss with you what your treatment options are and agree a treatment plan with you.
Before You Arrive
BEFORE you arrive for your FIRST APPOINTMENT we require you to bring the following information with you:
- Health and Care Number which can be found on a medical card, a hospital letter, prescription from GP, request from your doctor or A current list of your medication
- An accurate and up to date list of any medication you are taking
- Evidence of any Benefit you are receiving that may entitle you to free treatment under the Health Service
On Arrival
We ask that on your first visit you arrive 15 minutes prior to your allocated appointment time as you will be required to fill in a Private and Confidential Patient Questionnaire
After Your First Appointment
After your first appointment you will be registered with us and will be added to our six monthly recall system. We will post you an appointment card to your home address enabling you and your family to come and receive two appointments per year for a regular checkup by our care team. You will remain on our patient list for 24 months from your last visit with us.
